need info on 21st Century UH-1C "Hoghead II"

Can anyone provide any info as to whether or not the 21st Century UH-1C "Hoghead II" gunship is real helicopter or a fictional one?
If it's a real helicopter, can you provide the squadron number, pilot's name, etc.?
