I have this in storage. I'll have to dig it out and get some pics of it. Found it a few years ago on ebay and got it for a few bucks.

It looks like they are very hard to find now and sold out almost everywhere, so assuming this site (the manufacturer) still has them listed as available, it might be a good idea to pick one up direct.
